UnitedHealth Stock Analysis: Strong Q2 2026 Recovery and Higher Guidance
UnitedHealth Group (NYSE: UNH) delivered a much stronger second quarter than a year ago, with improved profitability, healthier medical cost trends, and an upgraded full-year earnings outlook. The results suggest that management's restructuring efforts and renewed focus on operational discipline are beginning to gain traction after a challenging period for the healthcare giant.

Second Quarter Results Show Improvement
Second-quarter revenue reached $112.0 billion, essentially flat from $111.6 billion a year earlier. While top-line growth remained modest, profitability improved significantly. Operating earnings climbed to $8.0 billion, up from $5.2 billion in the prior-year quarter (the second quarter 2025 includes $1.2 billion of unfavorable discrete impacts), while net income attributable to shareholders increased to $5.5 billion, compared with $3.4 billion a year ago. Diluted earnings per share rose to $6.04, and adjusted EPS came in at $6.38.
The medical care ratio—a critical metric for any managed care company—improved significantly to 86.7%, down from 89.4% in the year-ago quarter. Better aligned pricing, favorable member mix, product design changes, and medical cost management all contributed to the stronger margin profile. In addition, UnitedHealth benefited from $860 million of favorable prior-period reserve development.
The operating cost ratio increased modestly to 12.7%, reflecting ongoing investments in technology, artificial intelligence, digital infrastructure, care delivery, and customer experience initiatives.
Cash generation also remained exceptionally strong. Operating cash flow reached $11.1 billion, representing approximately 1.9 times net income, while the company's balance sheet continued to improve. The company ended the quarter with $31.5 billion in cash and short-term investments and $73.3 billion in total debt, while the debt-to-capital ratio tightened to 41.2%, moving steadily toward the long-term target of approximately 40% by year-end.
Segment Breakdown: UnitedHealthcare Rebounds, Optum Expands Margins
UnitedHealthcare generated $86.0 billion in revenue, compared to $86.1 billion in the second quarter 2025. UnitedHealthcare served 48.5 million people in the second quarter 2026, down 525,000 sequentially. Operating earnings rose to $3.9 billion, nearly doubling operating profit from the prior year. Improved pricing discipline, benefit redesign, and tighter medical cost management were the primary drivers of higher profitability.
Optum continued to be another major source of strength. Revenue reached $65.7 billion, while operating earnings increased to $4.0 billion, representing 160 basis points of margin expansion compared with last year. Optum Health improved operational performance despite serving fewer value-based care patients, Optum Insight benefited from AI-enabled healthcare technology solutions and completed the acquisition of Alegeus, while Optum Rx continued advancing its transparent pharmacy pricing model alongside improving profitability.
Strategic Initiative
Management also highlighted several strategic initiatives aimed at rebuilding trust and simplifying healthcare delivery. These include reducing prior authorization requirements, increasing transparency across pharmacy benefit management, expanding support for rural healthcare providers, investing $1 billion through the United Health Foundation, and introducing broader governance reforms. These actions are intended to improve affordability while strengthening long-term relationships with patients, providers, and regulators.
Company Raises Full-Year 2026 Outlook
The company raised its adjusted earnings per share outlook to $19.50–$20.00, up from the prior range of >$17.75. Operating earnings were guided above $25.45 billion, while cash flows from operations were projected to be approximately $24.00 billion. The medical care ratio forecast was declined to 88.1% (±25 bps), compared to the prior range of 88.8% (±50 bps).
The company also reaffirmed its commitment to at least $5.0 billion in share repurchases for the full year, having already completed $4.0 billion through mid-July 2026.
UNH Stock Technical Analysis
From a technical perspective, the $456 level has emerged as the near-term resistance for UNH stock. Should UNH clear and hold above $456 on conviction, the path opens toward the next upside target at $489.
However, failure to overcome the $456 resistance may trigger another period of consolidation. In that scenario, traders should watch the $384 area as the first major support. If selling pressure intensifies and that level fails to hold, the next significant support is located near $350.
